We are letting you know with mixed emotions that Dr. Shukla is going to retire as of 09/15/2026 thereafter this office will be permanently closed.

It has been a great pleasure serving health care needs of your children over the years, and we thank you for your loyalty and trust.

You can contact your insurance company to assist you to assign a new provider for continuity of care. Download release of records form, from patient forms tab and send it to us BEFORE 09/15/2026. 

Any request for record release AFTER 09/15/2026 should be submitted to Clary Document Company.
